Please download and complete the following forms. Several of these forms have changed. Make sure you have the latest version. They should be downloaded after January 12, 2012.All forms must be completed and turned in at Registration Night, January 25, 2012, along with your $250 registration fee, payable to Eisenhower Lacrosse.
